New Patient Registration
The practice accepts patients within a 3 mile radius of the surgery.
If you wish to register with The Surgery call in for the necessary forms. You will need to complete the following forms:
which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Exercise is the miracle cure we’ve always had, but we’ve neglected to take our recommended dose for too long. Our health is now suffering as a consequence. This is no snake oil. Whatever your age, there’s strong scientific evidence that being physically active can help you lead a healthier and even happier life.
