General Practice Data for Planning and Research (GPDfPR) – Data Collection

NHS Digital have recently announced it is setting up a new primary care data collection service which will begin on the 1st July 2021.

There are two parts to the data collection –

Type 1 includes GP practice records, if you wish to opt out please fill out the attached form and return this to your GP practice  Type 1 Opt out form

Type 2 includes national data, if you wish to opt out please visit the NHS digital website above, or call the helpline on 0300 303 5678 – Monday to Friday, 9am to 5pm (excluding bank holidays).
